2010 Columbia Invitationals applications are now open

The entrance fee must accompany the certificate of registration. The certificate must be postmarked and sent to the Championship Tournaments office on or before April 1, 2010 to guarantee consideration. All late applications will be put on a wait list for review. Upon acceptance, the fee is non-refundable.

If your team is accepted and later withdraws the non-refundable entry fee, sanctions will be placed against your team/club for future participation in Championship Tournaments events.

All applications will be online and no faxed applications will be accepted.

In order for the Tournament Committee to evaluate your application, it is imperative that each team submits correct/complete information.

Please note that if a club enters multiple applications for its member teams, each team will be evaluated individually and the tournament committee will make a decision on a per team basis and NOT a club basis.

Original 2009/2010 Player Passes; either USYS State Association or US Club Soccer passes

Original 2009/2010 Official Roster; either USYS State Association or US Club Soccer

Guest Players identified: Written on front or back of COPIES of Rosters

Player Passes for Guest Player

Individual Player Medical Release Forms

Permission to Travel Paper Work: Not Required for US Club Soccer Teams

USYS State Association teams from Region I do not need Permission to Travel Paper Work except teams from Connecticut or New Jersey.

USYS State Association teams from all other Regions need Permission to Travel Paper Work.
